Coverage Details:
Reimbursement Rate:Â 70% of eligible costs.
Lifetime Maximum:Â Up to $20,000 per person.
Eligible Treatments:
Residential Rehabilitation
For individuals undergoing inpatient rehabilitation programs.
Members must provide receipts and a certificate or letter confirming completion of the program to claim reimbursement.
Non-Residential Rehabilitation
For outpatient treatment programs.
In addition to receipts and proof of program completion, members must submit a letter from:
A physician,
The Employee and Family Assistance Program (EFAP) provider, or
A disability provider (Canada Life), confirming the appropriateness of the outpatient treatment.
